Introduction: In 1915, Austrian otologist Robert Bárány received extraordinary news whilst imprisoned at Przemyśl Fortress: he had been awarded the Nobel Prize in Physiology & Medicine. His journey from battlefield surgeon to Nobel laureate exemplifies scientific innovation amidst wartime adversity, with contributions that remain fundamental to modern neurotology.
Methods: Narrative historical review based on targeted searches of published medical literature, Nobel Prize archives and selected biographical sources.
Results: Bárány's 1906 discovery of the caloric test - observed when a patient complained irrigation water was "too cool" causing dizziness - revolutionised vestibular assessment by demonstrating temperature-induced endolymph currents. Despite disabling knee ankylosis from childhood tuberculosis, he volunteered for WWI service in 1914, subsequently advocating primary closure of cranial bullet wounds, work that preceded Neurosurgeon Dr Harvey Cushing's similar innovations. Captured during the Russian siege of Przemyśl, Poland in March 1915, one of the war's longest and most brutal engagements, Bárány learned of his Nobel Prize whilst imprisoned. Diplomatic intervention by Prince Carl of Sweden secured his release in 1916. He established Uppsala University's otology clinic in 1917, becoming full professor in 1926 and continuing innovations until his death in 1936. Today, the Bárány Society, founded in 1960, perpetuates his legacy through international meetings and research awards. His caloric test remains clinically essential alongside modern technologies in contemporary vestibular clinics worldwide.
